How to find a provider
Populate the search criteria above to find an Adventist Health provider.
California Members
Tier One providers must meet one of the following criteria:
In Tier one of the Incentive Health Provider Access Solution directory; or
In both the Adventist Health Medical Staff directory (use search criteria above) and included in Tier 2 of the Incentive Health Provider Access Solution directory; or
In the UC Davis Provider Network directory
Tier Two providers must meet the following criteria:
In the California Foundation for Medical Care (Tier Two) of the Incentive Health Provider Access Solution directory.
Tier Three providers must meet the following criteria:
In the Tier Three coverage tier of the Incentive Health Provider Access Solution directory. Members will be directed to HS technology to find an out-of-network provider.
*If traveling outside of California, utilize the First Health PPO network. No out of network coverage available for California associates traveling outside of California.
Oregon Members
Tier One providers must meet one of the following criteria:
